Contract Length: 13 weeks

Job Description

• Provide physical therapy services in acute care and inpatient rehabilitation settings.
• Must be comfortable performing acute care and inpatient rehab evaluations.
• Work schedule: Monday to Friday, 5 days a week, 8-hour shifts (8:00 AM - 4:30 PM).
• Weekend rotation every 6-7 weeks; when working a weekend, the PT receives a day off the week before and after.
• One holiday rotation per year.
• Attire: Caribbean blue or khakis with lab coat.
• Travelers must shave to ensure proper mask fit for respirator fit testing; use of personal PAPRs or CAPRs is not allowed.

Required Qualifications

• Valid Physical Therapist license in Illinois (license can be pending).
• Minimum 1 year of Physical Therapist experience within the last year.
• Experience in an Acute Care setting required.
• Experience with Epic electronic health record system required.
• Basic Life Support (BLS) certification required.

Required Certifications & Compliance

• BLS (Basic Life Support) certification (AHA accepted).
• Must complete OSHA Respirator Clearance prior to start to enable fit testing onsite.
• Fit testing will be completed onsite during the first week of orientation; failure to comply may result in removal from assignment.
• Physical exam within 1 year indicating good health (Aya standard).
• Negative drug screen within 12 months of start.
• TB test (PPD, Qgold, or TSPOT) within 12 months; chest x-ray within 5 years if prior positive PPD.
• Flu vaccine or approved medical/religious exemption required annually during flu season.
• COVID vaccination series preferred.
• Required immunizations or proof of immunity: Rubeola (Measles), Mumps, Rubella, Varicella, Hepatitis B (titer, series, or declination).
• Background check within 12 months including SSN trace, 7-year county and nationwide search, sex offender search.
• Completion of facility-specific orientation modules and documents prior to start.

• Must provide government-issued photo ID (driver’s license or state ID).
• Must complete confidentiality and security agreements prior to start.
• Must complete post-start orientation checklists during first week on assignment.
• Two professional references required (Aya standard for new candidates).
• Skills checklist required.

Work Environment & Notes

• Locals are accepted for this travel contract.
• No PAPRs or CAPRs provided or allowed for travelers; shaving is mandatory for mask seal.
• Facility does not keep fit test records; candidates must provide fit test documentation to agency.
• Modules and training must be completed annually or as required by unit.
