Job Opportunity: Care Coordinator (Registered Nurse)
Location: San Francisco, California
Employment Type: Travel/Contract
Shift: Day (4x10 hours), 7:30 AM – 6:00 PM
Contract Length: 24 weeks
Job Summary
- Coordinate care for patients with heart transplants, including:
- Immunosuppression monitoring
- Prophylactic antibiotic management
- Monitoring for organ rejection and infection
- Routine patient monitoring
- Ability to float to other facilities within the system
- Requires cardiac nursing experience, specifically managing heart failure and/or heart transplant recipients
Required Qualifications
- Valid Registered Nurse (RN) license in California
- Minimum 1 year of Case Management experience, including experience within the last year
- Basic Life Support (BLS) certification issued by the American Heart Association
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Experience in an outpatient setting
- Transplant experience
- Ambulatory care experience
