Physical Therapist - Home Health (Travel/Contract)

Location: Sarasota, Florida
Contract Length: 20 weeks
Shift: Day shift, 8:00 AM – 5:00 PM (5 days/week, 40 hours/week)

Job Description

Provide skilled home health physical therapy visits (6-8 visits per day) for patients with conditions including med/surg, cardiac, ortho (hips/knees/shoulders), post-surgical, COPD, CHF, pneumonia, CVA, Parkinson’s, trauma, amputations (AKA, BKA), respiratory patients, neuro, weakness, falls, and failure to thrive.
Perform assessments and treatments involving cardiopulmonary/circulatory systems, orthopedic and neurological rehabilitation, use of equipment and devices (gait belt, rolling walker, sit-to-stand lift, sliding boards, collars, supplemental oxygen, prosthetics, wheelchair assessment).
Develop and implement home exercise programs.
Provide patient and family education, including safety and injury prevention.
Document thoroughly using electronic systems.
Participate in billing processes.
Weekend work required 1-2 weekends per contract.

Required Qualifications

Valid Physical Therapist license in Florida.
Minimum 1 year of Physical Therapist experience, including experience within the last year.
Experience in a Home Health setting.
Basic Life Support (BLS) certification.
