Job Overview
- Location: Springfield, Ohio
- Employment type: Travel/Contract
- Shift: Days, Monday–Friday, 8:00 AM–5:00 PM
- Hours: 40 per week
- Contract length: 13 weeks
Setting and Caseload
- Home care setting
- Average daily patient volume: 8
- Typical productivity expectation: minimum 6.5 points, usually 3–4 evaluations
- Common patient population: mostly post-acute
- Primary service areas: Clark, Edgar, Effingham, Champaign, and Madison County
- Therapist schedules their own day and can move visits around
- One COTA on site who will need supervision
Required Qualifications
- Valid Occupational Therapist license in Ohio
- BLS issued by American Heart Association or American Red Cross
- Experience in a Home Health setting
- Home care home base experience
Certifications
- BLS
- CPR
Schedule and Work Expectations
- No weekend requirement
- No holiday requirement
- Guaranteed 40 hours per week

- Government-issued photo ID
- Emergency contact form before start
- Background check
- EPLS/GSA/SAM screening
- HHS/OIG screening
- Physical exam within 1 year of start
- Negative TB test or chest X-ray within 1 year of start
- Measles, mumps, rubella, hepatitis B, and varicella immunity/documentation
- Drug screen within 1 year of start
- Flu vaccination or declination during flu season
- 2 professional references within 3 years of start
