Job Opportunity: Physical Therapist (Outpatient) - Travel/Contract

Required Qualifications

• Years of Experience: Minimum 1 year in specialty
• Certifications:

  • Basic Life Support (BLS) - ARC, AHA, or US Department of Defense approved
• Licensure: Valid Physical Therapist license for Texas (Primary Source Verification required)

Additional Requirements

• Prior travel experience not explicitly required
• Locals allowed and first-time travelers accepted
• EMR system: Meditech

Compliance & Documentation

• Proof of I9 or Attestation
• Government issued photo ID (must be carried during hours of service)
• Proof of E-Verify or Attestation
• Background checks including:

  • 7-year criminal check within a year of start, updated annually
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start and updated for renewals
• Education verification upon request
• Drug screening: 9 Panel Drug Screen within a year of start (instant drug screen acceptable)
• Physical exam: Physician statement within last 12 months (not required annually)
• Immunizations and health requirements:

  • Negative TB test or chest X-ray within 1 year (updated annually)
  • Measles, Mumps, Rubella: 2 vaccines or positive titer (exempt if born before 1957)
  • Hepatitis B: Titer, vaccine series, or declination accepted
  • Varicella: 2 vaccines, titer, or history of chickenpox
  • Seasonal flu vaccine preferred but not mandatory; mask required if declined
  • COVID-19 vaccination proof required prior to start; religious/medical exemptions considered
• N95 Fit Test within 1 year of start
• References: 2 professional references within 2 years required for travel positions
• Competency exams and medication/pharmacology competency as applicable

Notes

• Floating may be required within the scope of practice
• No call responsibilities
• Orientation and safety manual acknowledgment required
• Various certifications such as ACLS, PALS, NRP, TNCC may be required as applicable (refer to unit specifics)
