Required Qualifications

• Valid RN license in Oregon
• Preoperative (2 years of experience)
• Preoperative experience within the last year
• One completed travel assignment
• BLS issued by American Heart Association
• ACLS issued by American Heart Association
• PALS issued by American Heart Association
• Associate of Science in Nursing / Associate Degree in Nursing
• Experience with Epic

• 7-year criminal background check
• EPLS/GSA/SAM check
• HHS/OIG check
• Education verification
• International background check, if the traveler has held residency outside the US in the past 10 years
• Orientation Safety Manual acknowledgement
• Medication Test/Pharmacology Competency Exam
• Competency Exam
• Physical exam within 1 year prior to start
• Fit test on site
• Tdap vaccination within 10 years prior to start, or signed declination
• Negative TB test or chest X-ray within 1 year prior to start
• Rubeola immunity documentation
• Mumps immunity documentation
• Rubella immunity documentation
• Hepatitis B immunity documentation, vaccination series, or signed declination
• Varicella immunity documentation
• 10-panel drug screen with methamphetamines listed out
• Flu vaccination or signed declination during flu season
• At least 2 professional employment references
• Skills checklist
