Physical Therapist - Outpatient Rehab

Location: Taos, New Mexico
Employment Type: Travel/Contract
Contract Length: 13 weeks
Schedule:

• Primarily Sun-Thurs, 5 days per week, 8-hour shifts (8:00 AM - 4:00 PM)
• Option/preference for 4x10 schedule can be discussed

Job Description

• Outpatient physical therapy with occasional inpatient coverage for illness or vacation
• Caseload ideally includes orthopedic, neurological, vestibular, and pelvic health patients
• Work primarily outpatient with some inpatient responsibilities as needed

Required Qualifications

• Valid Physical Therapist license in New Mexico

Documentation & Compliance

• Provide professional photo for badge (clear, close-up, solid background)
• Complete facility-specific forms prior to start, including:

  • TB questionnaire
  • Code of conduct
  • Confidentiality agreement
  • Behavior standards
  • Emergency contact information
  • Policy and procedure acknowledgments
  • IT guidelines
  • Orientation attestation (including watching required videos)
  • COVID and influenza declination forms if applicable
• Employment application/resume with 7-year work history including locations, gaps, and end dates in MM/YYYY format
• Proof of 2 clinical references or hospital evaluations
• Performance evaluations if previously worked with the facility for over 1 year

• 75-mile radius restriction for local candidates
• Prior travel experience is not explicitly required for this position
• Orientation and onboarding include mandatory education and safety training
