Job Opportunity: Pharmacist Inpatient

Location: Bend, Oregon
Employment Type: Travel/Contract
Contract Length: 8 weeks


Job Description

Relief Pharmacist position covering recruiting, hiring, training, and leave coverage. Full-time, 40 hours per week.


Required Qualifications

  • Valid Hospital Pharmacist license in Oregon
  • Minimum of 1 year of hospital pharmacist experience within the last year

Background Checks & Verifications

  • 7-year criminal background check (including SSN trace, counties lived/worked/school, FACIS L3, DOJ sex offender search, nationwide check).
  • EPLS/GSA/SAM check.
  • HHS/OIG check.
  • Education verification (highest level) with diploma or transcript on file.
  • Primary source verification of state license.

Competencies

  • Medication Test/Pharmacology Competency Exam as applicable.
  • Competency exam as applicable by profession/specialty.

Facility-Specific Requirements

  • Passport-style photo for badge within 48 hours of signing and no later than 2 Wednesdays prior to start.
  • Completion and signing of confidentiality, contractual access, compliance/privacy, and emergency contact forms within the same timeframe.

Health Requirements

  • Physical exam within 1 year of start.
  • TDAP vaccine within 10 years of start (vaccine after age 19; declination accepted).
  • Negative TB test (PPD, QGold, or TSPOT) within 1 year; for positive responders, negative chest X-ray within 5 years and annual TB questionnaire required.
  • Annual TB questionnaire.
  • Immunity or vaccination (or declination) for:
    • Measles (Rubeola)
    • Mumps
    • Rubella
    • Hepatitis B (positive titer required; vaccine series alone not acceptable)
    • Varicella
  • Negative 12-panel drug screen within 90 days of start (not required annually).
  • Seasonal flu vaccination or agency declination/exemption required during flu season.
  • COVID vaccine preferred but not required; declination or exemption required if never vaccinated.
  • Respiratory safety clearance required within 1 year of start and annually.
