Specialty Experience: Minimum 1 year hospital experience with GE scanners

Certifications & Licenses Required

BLS (AHA certified)
ARRT CT certification
State of Wisconsin license
Current Driver’s License/State ID (must not be expired and address must comply with compact license rules)

Skills & Systems

Experience with EPIC electronic health record system
Ability to operate GE CT scanners

Health & Compliance Requirements

MMR: Documented proof of two vaccines or positive IGG titer (no declinations accepted)
Fit Test: Required annually; preferred before start, can be completed onsite
Hepatitis B: Vaccination series completion or signed declination
Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
Positive PPD: Negative chest X-ray and annual TB questionnaire if applicable
Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually
2 Step PPD or IGRA: Required prior to start and annually thereafter
Varicella: Proof of immunity or vaccination (no declinations accepted)
Tdap: Valid within 10 years, no declinations accepted
12 Panel Drug Screen: Within 30 days prior to start and annually thereafter

Onboarding & Documentation Requirements

Signed Assignment Agreement (required before onboarding)
Ascension Resume/Profile and 2 Professional References (required for submission)
License/Certificate Verification (current and primary source verified within 30 days prior to start)
Ascension Core Competencies (HIPAA, Joint Commission, OSHA topics; minimum 80% score; updated yearly)
Ascension Medical Imaging Contract Worker Onboarding Education Attestation (6 hours pre-hire training attestation)
Criminal Background Check (including SSN trace, employment verification, OIG search, national sex offender registry; updated annually)
EPLS check (updated within 90 days of start)
Skills Checklist (clinical, updated annually)
Education Verification (relevant to clinical work, updated within 90 days of start)
Employment Verification (past 7 years, updated annually)
Confirmation of Assignment (start/end dates on agency letterhead)
