Job Description

Care coordination, utilization review, and discharge planning
Computer literacy required

Required Qualifications

Valid Registered Nurse (RN) license in New York
Minimum 5 years of case management experience, including experience within the last year
PRI and Screen certification
AHA Basic Life Support (BLS) certification issued by the American Heart Association
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN) from an accredited program
Experience in care coordination, utilization review (InterQual), and discharge planning
Emergency Department (ED) experience
