Job Description

• Review inpatient and/or outpatient medical records to ensure accurate representation of severity of illness and proper coding.
• Validate that coding reflects medical necessity and accurately reports patient illness severity, risk of mortality, and complexity of care.
• Ensure discharge diagnoses and co-morbidities fully reflect the patient’s clinical status and care.
• Use advanced knowledge of disease processes, medications, and critical thinking to identify documentation gaps.
• Identify opportunities for improving clinical documentation in concurrent and retrospective inpatient reviews to support quality coding.
• Apply regulatory compliance related to documentation, coding, and billing for all health insurance plans.
• Collaborate extensively with physicians, coding staff, case management, nursing, and other caregivers to facilitate documentation modifications.
• Act as an educator and resource to improve quality and completeness of clinical documentation.
• Perform duties in compliance with ethical, legal, federal, and state regulations, official coding rules, and accepted standards of coding practice.
• Must be able to work flexible hours, including evenings and weekends as needed.

Required Qualifications

• Valid RN license in Minnesota
• Minimum 3 years of experience as a Clinical Document Improvement Specialist, including experience within the last year
• Certification: CDIP or CCDS required
• Valid certification/license: RHIT, RHIA, CCS, or CCS-P required
• Associate of Science in Nursing or Associate Degree in Nursing
• Knowledge and application of regulatory compliance related to documentation, coding, and billing for all health insurance plans
