Physical Therapist for Home Health setting
Home Health experience preferred
Oasis experience required; candidates without Oasis experience may be considered if they have solid Home Health experience
Hours based on completed visits (e.g., 25 visits = 40 hours)
COVID exemptions permitted
Mileage reimbursed at IRS standard rates
Accepting candidates with pending California PT license (must have applied upon submission)

Basic Life Support (BLS) - AHA Instructor or Provider acceptable
Advanced Cardiac Life Support (ACLS) - as applicable
Other certifications (PALS, NRP, EFM) as applicable
Valid government-issued photo ID (Driver’s License or State ID)
Passport copy required before start
Background checks including 7-year search, EPLS/GSA/SAM, HHS/OIG (updated annually)
Physical exam within 12 months of start
Immunizations and titers as applicable:
- Tetanus/Diphtheria/Tdap (within 10 years, no declinations accepted)
- Negative TB test or chest X-ray within 1 year (with special rules for past positives)
- Measles, Mumps, Rubella (MMR) proof or titers for candidates born in 1957 or later
- Hepatitis B titer or vaccine series (declination accepted if within 1 year)
- Varicella titer, vaccine, or history of chickenpox (declinations not accepted)
- Seasonal flu vaccination/documentation (declinations accepted)
- COVID vaccination proof or medical/religious exemption required
Negative 9-panel drug screen within 1 year (instant drug screen acceptable)
Photo for badge (passport style)
Personal or auto insurance and auto registration (updated while on assignment)
Employment application and skills checklist (annual)
