Job Opportunity: PT Home Health (Travel/Contract)
Location: Glendale, California

Job Description
- Physical Therapist for Home Health setting
- Home Health experience preferred
- Oasis experience required; candidates without Oasis experience may be considered if they have solid Home Health experience
- Hours based on completed visits (e.g., 25 visits = 40 hours)
- COVID exemptions permitted
- Mileage reimbursed at IRS standard rates
- Accepting candidates with pending California PT license (must have applied upon submission)
Required Qualifications
- Valid Physical Therapist license in California
- Minimum of 1 year of Physical Therapist experience
- Physical Therapist experience within the last year
Required Certifications & Documents
- Basic Life Support (BLS) - AHA Instructor or Provider acceptable
- Advanced Cardiac Life Support (ACLS) - as applicable
- Other certifications (PALS, NRP, EFM) as applicable
- Valid government-issued photo ID (Driver’s License or State ID)
- Passport copy required before start
- Background checks including 7-year search, EPLS/GSA/SAM, HHS/OIG (updated annually)
- Physical exam within 12 months of start
- Immunizations and titers as applicable:
- Tetanus/Diphtheria/Tdap (within 10 years, no declinations accepted)
- Negative TB test or chest X-ray within 1 year (with special rules for past positives)
- Measles, Mumps, Rubella (MMR) proof or titers for candidates born in 1957 or later
- Hepatitis B titer or vaccine series (declination accepted if within 1 year)
- Varicella titer, vaccine, or history of chickenpox (declinations not accepted)
- Seasonal flu vaccination/documentation (declinations accepted)
- COVID vaccination proof or medical/religious exemption required
- Negative 9-panel drug screen within 1 year (instant drug screen acceptable)
- Photo for badge (passport style)
- Personal or auto insurance and auto registration (updated while on assignment)
- Employment application and skills checklist (annual)
- Professional references (2 required)

- Hours guaranteed if 25 or more visits are completed
