Physical Therapist (SNF) - Travel/Contract Opportunity

Location: Live Oak, California
Assignment Length: 13 to 26 weeks
Shift: Day shift, 5 days per week, 8 hours per day (40 hours/week)
Start Date: ASAP

Job Description

• Provide physical therapy services in a Skilled Nursing Facility (SNF) setting.
• May include occasional weekend coverage and floating as needed.

Required Qualifications

• Valid Physical Therapist license in California.
• Minimum 1 year of Physical Therapist experience, including experience within the last year.

Compliance and Documentation Requirements

• Government-issued photo ID (Driver’s License or State ID).
• Emergency contact information prior to start.
• Background checks including 7-year residential/work/school history, sex offender registry, and annual federal exclusions (EPLS/GSA/SAM, HHS/OIG).
• Physical exam within 1 year of start date.
• Negative TB test (PPD, Quantiferon Gold, or T-spot) within 1 year; if past positive, a negative chest X-ray within 1 year is required.
• Immunization or titer proof for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B (titer, declination, or vaccine series)
  • Varicella (titer, vaccine series, or history of chickenpox)
• 9-panel drug screen within 1 year of start.
• Seasonal flu vaccination or declination during flu season (October 1 - March 31).
• Annual skills checklist and medication/pharmacology competency exam as applicable.

References

• 2 professional references required for travel, allied, or per diem acute care roles.
• 1 professional reference required for per diem non-acute care roles.
• References should be within 3 years of start date.

Additional Notes

• This is a travel/contract position requiring flexibility for occasional weekend shifts and floating duties.
• Prior travel experience is not explicitly required.
