Physical Therapist - Home Health (Travel/Contract)

Location: San Jose, California
Contract Length: 16 weeks (4 months)

Job Overview

• Specialty: Physical Therapy in Home Health
• Patient Focus: Home Health, Wound Care, Geriatric patients
• Visits: Minimum 23-24 visits per week required to qualify for a 40-hour guarantee
• Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 5:00 PM)
• Weekend Work: Not required
• On-Call: Not required (discretionary but not mandatory)
• Locals: Accepted within a 50-mile radius
• Mileage Reimbursement: IRS standard from first to last patient (not to/from home)
• Dress Code: Any color scrubs, closed-toe shoes
• Charting: Must be completed onsite using Kinnser/Wellsky; no remote charting allowed
• Parking: Free residential parking available

Required Qualifications

• Valid Physical Therapist license in California
• Minimum 1 year of Physical Therapist experience within the last year
• Experience in a Home Health setting
• Basic Life Support (BLS) certification

Additional Information

• First-time travel therapists are welcome to apply
• Contract length is non-negotiable: less experienced clinicians must commit to 6 months
• Compliance documents must be completed and approved before starting patient care (may take 2-3 weeks)
• All required Return to Office (RTO) documentation must be submitted with application; no additions accepted after offer
• Territory and county coverage details will be discussed during the interview; may include surrounding counties
• Valued clinician qualities: patient-centered care, punctuality, self-motivation, and thorough charting completion
