Licensed Practical Nurse (LPN) - Rehabilitation
Location: Utica, New York
Employment Type: Travel/Contract
Contract Length: 13 weeks
Shift: Evening (5 shifts per week, 7.5 hours each, 2:30 PM - 11:00 PM)
Job Description
- Provide nursing care in a rehabilitation and long-term care (LTC) nursing center.
- Responsible at times for passing medications to approximately 40 established long-term care residents.
- Work schedule includes every other weekend.
- Experience with PointClickCare electronic medical records (EMR) preferred.
- Local candidates within a 75-mile radius are accepted.
Required Qualifications
- Valid LPN/LVN license in New York.
- Minimum 1.5 years of LPN/LVN experience, including experience within the last year.
- Experience in rehabilitation and long-term care settings.
- Current Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.
- Pediatric Advanced Life Support (PALS) certification issued by the American Heart Association.
Required Documentation & Compliance
- Government-issued photo ID (Driver’s License or State ID).
- Emergency contact form completed prior to start.
- Background checks including 7-year work/school history, sex offender registry, and federal exclusions (updated annually).
- Education verification and Florida Education Audit Confirmation (for RN/LPN staff).
- Physical exam within 1 year of start date.
- Fit mask test within 1 year of start and updated annually (N95 or equivalent respirators).
- Negative TB test or chest X-ray within 1 year of start (PPD, Quantiferon Gold accepted; Tspot not accepted).
- Immunization or positive titer proof for:
- Measles (Rubeola)
- Mumps
- Rubella
- Varicella (or documented history of chickenpox)
- Hepatitis B (titer, declination, or vaccine series)
- Seasonal flu vaccination or declination during flu season (September 1 - March 31).
- Pneumococcal vaccine or signed declination required for all rehab travelers.
- 9-panel drug screen within 30 days of start (instant drug screen acceptable).
- Completion of facility-specific documents including code of conduct, sexual harassment prevention training, job description acknowledgment, and other orientation paperwork.
- Two professional references required within 3 years of start date (travel, allied, or per diem acute care).
Additional Notes
- Candidates must complete online learning modules prior to starting.
- Photo for badge must be a professional headshot (shoulders up, plain background, no head coverings or informal gestures).
- Medication competency and EKG competency exams may be required depending on assignment area.
- Prior travel experience is not explicitly required for this position.
