Job Opportunity: Home Health Registered Nurse

Location: Portland, Oregon

Job Details

Shift: Day shift, 8 hours per day (8:00 AM – 4:30 PM), 5 days a week (40 hours/week)
Start Date: ASAP
Patient Ratio: 1:1, with up to 6 patient visits per day (30 productivity points standard)
Weekend Requirement: No weekends required
On-Call: Not required
Floating: No floating between units
Block Schedule: Not accommodated
Locals Accepted: Yes, within 50-75 mile radius
Guaranteed Hours: Facility may call off up to one shift every two weeks
Transportation: Traveler must have their own transportation; daily travel radius up to ~60 miles (60-90 minutes total drive time)

Required Qualifications

Valid Registered Nurse (RN) license in Oregon
Minimum of 1 year of Home Health nursing experience within the last year
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Basic Life Support (BLS) certification issued by the American Heart Association
Experience with OASIS documentation in a Home Health setting

Compliance & Background Requirements

Government-issued photo ID and Social Security Card required
Background check within 90 days covering all locations lived/worked in past 7 years, including:
- SSN Trace, Death Master Index, Nationwide and County checks, National Sex Offender Registry, FACIS Level 3, Education Verification, Motor Vehicle Record (MVR)
State-specific searches and verifications as applicable for Oregon and Washington
Education verification required for nursing degree
Drug screening: 9-panel drug screen within 12 months of start
Physical exam within 1 year of start, signed by MD, NP, PA, or DO, indicating fitness for duty
Immunizations and health screenings required:
- TDAP within 10 years (declination accepted)
- Negative TB test or chest X-ray within 12 months (annual updates required)
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Seasonal flu vaccine during flu season (declination accepted)
Respirator clearance required annually for travel candidates
Fit mask testing required for travel candidates (annual renewal)

Additional Requirements

Minimum of 2 professional references covering 1 year of employment within the last 3 years in a comparable specialty/setting
Employment application covering at least the last 5 years of employment
Must have a valid driver’s license and reliable transportation
No weekend or on-call requirements
No floating between units

Prior Travel Experience

First-time travelers accepted (no prior travel experience required)

Notes

Facility cannot accommodate 4x10 or 3x12 shifts
Confirm return-to-office (RTO) restrictions with hiring manager
Background checks and other compliance documents must be current and uploaded as required
Annual regulatory and specialty-specific training modules are generally not required for Home Health at this facility
