Physical Therapist (PT) - Skilled Nursing Facility (SNF)

Location: Wenatchee, Washington
Employment Type: Travel/Contract
Assignment Length: Approximately 26 weeks (6 months preferred, open to shorter)
Shift: Day shift, 5 days a week, 8 hours per day (9:00 AM – 5:00 PM)
Unit: Skilled Nursing Facility (SNF)

Job Summary

Seeking a Physical Therapist to provide therapy services in a skilled nursing facility setting.

Required Qualifications

Valid Physical Therapist license in Washington.
Minimum 1 year of experience as a Physical Therapist.
Physical Therapist experience within the last year.

Compliance and Documentation Requirements

Government-issued photo ID (Driver’s License or State ID).
Emergency Contact Form (required prior to start).
Background checks within 1 year of start, including SS Trace, County searches (7 years), Nationwide, and Sex Offender registry. Annual updates required for EPLS/GSA/SAM and HHS/OIG checks.
Physical exam within 1 year of start.
Negative TB test (PPD, Quantiferon Gold, or T-spot) within 1 year; if past positive, a negative chest X-ray within 1 year is required. Annual TB questionnaire also required.
Immunization or titer proof for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, 2 vaccines, or history of chickenpox)
9 Panel Drug Screen within 1 year of start (instant drug screen acceptable).
Seasonal flu vaccination or declination required during flu season (October 1 – March 31).
Professional references:
- 2 for travel, allied, or per diem acute care roles.
- 1 for per diem non-acute care roles.
Annual skills checklist and competency exams as applicable.

Additional Information

No specific scrub color required.
Facility is located along the Columbia River, approximately 24 miles from Leavenworth, WA.

Hours

40 hours per week, 8 hours per day, Monday through Friday.
