Occupational Therapist - Home Health (Travel/Contract)

Location: San Jose, California
Contract Length: 13 weeks

Job Details

Employment Type: Travel/Contract
Shift: Day shift, 8 hours per day (7:00 AM - 3:00 PM), 5 days a week
Guaranteed Hours: 36 hours weekly
Patient Load: 5-6 patients per day (1:1 ratio, may visit up to 6 patients daily)
Weekends: Required if needed
On-Call: Not required
Locals Accepted: Yes
Transportation: Must have own transportation; occasional travel radius up to 50+ miles
Attire: Business casual or scrubs
Parking: Free
Mileage Reimbursement: IRS standard

Required Qualifications

Valid Occupational Therapist license in California
Minimum 1 year of Occupational Therapist experience within the last year
One completed travel assignment as an Occupational Therapist
Experience in a Home Health setting
Experience with OASIS
Basic Life Support (BLS) certification issued by American Heart Association or American Red Cross

Documentation & Compliance Requirements

Government-issued photo ID (Driver’s License/State ID)
Background checks within 1 year, including SS Trace, County searches (7 years), Nationwide, Sex Offender registry
Annual EPLS/GSA/SAM and HHS/OIG checks
Certifications: BLS required; ACLS and PALS preferred (American Heart Association or American Red Cross)
Physical exam within 1 year of start
Negative TB test or chest X-ray within 1 year (with specific guidelines for past positives)
Immunizations or proof of immunity for Measles, Mumps, Rubella, Hepatitis B, Varicella
9-panel drug screen within 1 year of start
Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31)
Professional references: 2 for travel/allied/per diem acute care; 1 for per diem non-acute care (within 3 years)
Annual skills checklist and competency exams as applicable

Additional Notes

Documentation tools will be provided and must be returned
The system groups clients to minimize drive and windshield time
Facility uses Home Health MD EMR system
