Valid RN license in Oklahoma
Med Surg / Telemetry (2 years of experience)
Med Surg / Telemetry experience within the last year
BLS issued by American Heart Association
NIHSS
Nursys verification within 30 days prior to start
Current license and primary source verification within 30 days prior to start of assignment
At least two professional references
Resume or profile
Skills checklist completed within one year of start and updated annually for specialty

Pharmacology exam: minimum 80%, completed prior to start, updated every 3 years
Specialty exam: minimum 80% or national benchmark passing score, completed prior to first assignment, updated every 3 years
Color vision completed prior to start and updated every 3 years
Core competencies completed within a year of start and updated every 3 years
NIH Certification updated yearly for Group A and every 2 years for Group B-F
