Pharmacist - Contract Position
Location: Glenwood Springs, Colorado
Employment Type: Contract
Contract Length: 20 weeks
Schedule
- Rotating day/evening shifts, 40 hours per week
- Start times: 6:30 AM or 8:00 AM, Monday through Friday
- Rotating weekends working 7:00 AM to 12:00 PM
Job Summary
- Prepare and dispense medication orders per physician requests
- Maintain medication stock in patient care areas
- Ensure safe, appropriate, and cost-effective medication therapies according to policies and protocols
- Support facility EMR conversion from Meditech to Epic (Epic experience required)
Primary Duties and Responsibilities
- Interpret medication orders and maintain computerized patient medication profiles
- Compound and dispense pharmaceuticals, including IV admixtures, chemotherapy, and parenteral nutrition
- Comply with federal, state, and local laws regarding controlled substances and maintain required records
- Monitor drug therapy regimens for contraindications, interactions, allergies, and appropriateness
- Perform pharmacokinetic drug dosing per protocol
- Review culture and sensitivity data for antibiotic appropriateness and recommend changes
- Extract and interpret patient chart information accurately
- Detect and report adverse drug reactions
- Promote formulary adherence and therapeutic substitution protocols
- Provide clinical consultation and drug information to practitioners, patients, and families
- Document all clinical activities and interventions accurately
- Participate in quality improvement and medication review activities
- Collect data, conduct quality monitors and inspections, and maintain documentation
- Assist in orientation, education, and training programs for pharmacy, nursing, and medical staff
- Serve as a member of the Code Response Team
Required Qualifications
- Active Colorado Pharmacist License (PHA) in good standing
- Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy degree
- BLS (American Heart Association) certification
- ACLS (American Heart Association) certification
- Flu vaccine required (no exemptions) from October 1 through April 1
- Epic experience required
