Surgical Technician (Certified Surgical Technologist)

Location: Des Moines, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: 7:00 PM – 7:30 AM (overnight)
Weekend/Holiday Requirements:

• Every third weekend shift required
• Every other holiday required
• No on-call required
• Must be willing to float to West Des Moines if needed

Required Qualifications

• Certified Surgical Technologist (CST) certification (NBSTSA Certification)
• Minimum of 1 year of experience as a Certified Surgical Technologist, including experience within the last year
• American Heart Association Basic Life Support (BLS) certification

Professional and Compliance Requirements

• Completed profile including resume, skills checklist (within one year), and 2 professional references
• Signed Confidentiality of Patient & Proprietary Information and System Security Agreement
• Signed Conflict of Interest Disclosure Form
• Federal, nationwide, and county criminal background check covering last 7 years, including SSN verification (dated within 30 days of start)
• Education verification of highest academic degree or diploma/GED
• EPLS (Excluded Parties List System) check dated within 30 days
• OIG (Office of Inspector General) exclusion check dated within 30 days
• Integrity and Audit Services Form signed shortly before start
• Non-employee upload form specific to Iowa location completed and uploaded
• NSO (National Sex Offender) check dated within 30 days, including all aliases
• System Access Form completed and uploaded one week before start
• Child and Adult Dependent Abuse Training (Iowa only)
• Code of Conduct Form signed
• Standard I-9 verification completed by agency
• Iowa-specific criminal history, sex offender, and abuse registry checks completed within 30 days prior to start
• OFAC (Office of Foreign Assets Control) sanctions check completed within 30 days
