Required Experience

Health and Immunization Requirements

• COVID-19 Vaccine and 1 booster or approved exemption
• MMR: Proof of 2 immunizations or positive IGG titer; if negative titer, booster or declination accepted
• Hepatitis B: Proof of 3 immunizations and positive antibody titer; if negative titer, booster or declination accepted
• TB Test: Negative test within 30 days of start (skin test, QuantiFERON Gold, or T-Spot); if positive, proof of history and chest x-ray within 5 years plus annual questionnaire
• Varicella: Proof of 2 immunizations, positive titer, or declination accepted
• Tdap: Proof of vaccination within past 10 years or declination accepted
• Influenza Vaccine: Required October - March unless medical/religious exemption approved
• Drug Screen: 9 panel minus THC within 30 days of start, updated annually; rapid drug screen accepted

Background and Compliance Checks

• 7-year County, National Criminal Background Check, SSN Trace, Employment Verification, OFAC, NSO within 30 days of start; updated every 3 years
• Manual searches within 30 days of start and updated annually for:

  • OIG
  • EPLS
  • OFAC
  • National Sex Offender
• CA state healthcare exclusion list check (Medi-Cal Suspended and Ineligible Provider List) within 30 days prior to first assignment and monthly thereafter
• CHS Confirmation of Assignment/Job Description signed on start date
• CHS Acknowledgement of Receipt for Policy Packet signed on start date
• CHS Dispute Resolution Agreement signed on start date
• CHS Confidentiality of Receipt signed on start date

Additional Requirements

• Fit Test will be completed onsite on start date (optional)
• Driver's License/State ID required
• Resume/Profile submission required
• Two professional references required (minimum one supervisory)
• CHS Skills Checklist (clinical skill-specific) to be completed within 1 year of start
• Badge photo with neutral background, no facial jewelry
• CHS ROI for MyChart required
