RN Case Manager

Location: Nashville, Tennessee
Shift: 08:00 - 16:30
Contract length: 12 weeks

Role Summary

Coordinates, plans, and facilitates services for individuals by assessing needs, developing and updating care plans, monitoring progress, and collaborating with interdisciplinary teams.

Required Qualifications

Valid RN license in Tennessee
Case Management experience within the last year
1 year of Case Management experience
Associate of Science in Nursing / Associate Degree in Nursing
Driver's License
