Case Manager

Overview

• Temp role in Pittsfield, Massachusetts
• 13-week assignment
• Days, 8-hour shifts
• Must work five 8-hour shifts; 10/12-hour shifts are not available
• Travelers may not take all holidays off and must work two holiday blocks if scheduled/needed:

  • Thanksgiving Day/Friday after and the weekend after
  • Christmas Eve through Christmas Day and the weekend after
  • New Year’s Eve through New Year’s Day and the weekend after
• Requested time off must be noted and verified at upload; no more than 5 days of RTO will be accepted
• Travelers within 50 miles of the facility are not considered
• Traveler must have their own vehicle

Required Qualifications

• Valid RN license in Massachusetts
• Associate of Science in Nursing / Associate Degree in Nursing
• Active American Heart Association or American Red Cross BLS
• 3 years of Case Management experience
• Case Management experience within the last year
• Experience in an Acute Care setting
• Acute Hospital Case Management Experience
• Experience in a Home Health or Long Term Care or Rehabilitation setting
• Driver's License
• Discharge Planning
• Internal Medicine experience
• Orthopedics experience
• Cardiology experience
• Oncology experience
• Massachusetts Temporary Nursing Service Agency License (Nursing Pool) must be included up front with the submission

Preferred Qualifications

• Data management skills
• 3+ years clinical experience in Rehab Setting, Home Health, and Long-Term Care
• Knowledge of regulatory and accreditation organizations such as Joint Commission, DPH, and HealthGrades
• Meditech Expanse experience
• CCM or ACM
• RN-BC in Case Management
