Requirements

• Licensure: NY LPN license in hand
• Experience: Hospital experience within the last 2 years
• Ability: Must be able to take own patient assignments with RN support
• Prior Travel Experience: Not explicitly required

• ACLS (AHA Provider/Instructor/RQI acceptable)
• BLS (AHA Provider/Instructor/RQI acceptable)
• PALS (AHA Provider/Instructor/RQI acceptable)
• NRP (Neonatal Resuscitation Procedure)
• TNCC (Trauma Nursing Core Course)
• EFM (Electronic Fetal Monitoring)
• State License Verification (Primary Source Verification within 30 days of start)
• NURSYS Verification (for LPN/LVN within 30 days of start)
• NIH Stroke Certification (if required by unit)
• Tech Cert Verification (if applicable within 30 days of start)
• Orientation Safety Manual Acknowledgement (required)

Background and Compliance Checks

• 7-year criminal background check (felony checks) within 3 months of start
• EPLS/GSA/SAM check within 3 months of start and monthly thereafter
• HHS/OIG check within 3 months of start and monthly thereafter
• Education verification from primary source (no hard copy degrees accepted)
• Foreign equivalency education evaluation if applicable
• Transcript/Diploma if education verification is not on file

Health and Immunization Requirements (Must be completed within 5 days of contract signing)

• Physical exam within 1 year of start (updated annually)
• Fit mask test within 1 year of start (Halyard duckbill 1870+1860 Regular or 1860 Small)
• Negative TB test (Qgold, Tspot, or 2-Step PPD) within 1 year of start or Chest X-Ray if positive
• TB Questionnaire (in lieu of annual TB test renewal)
• Immunizations or positive titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (Surface AB positive titer, completed series or declination)
  • Varicella
• Drug screening within 30 days of start (includes Amphetamines, Cocaine, Ecstasy, Fentanyl, Heroin, Opiates)
• Seasonal Flu vaccination or signed declination (required during flu season)
• Eye exam (Snellen) within 1 year of start (not required annually)
• COVID vaccination copy preferred but not required

Competency Requirements

• Medication Test/Pharmacology Competency Exam (as required by profession)
• Color Blind Testing (required once, not annually)
• Competency Exam (as required by profession/specialty)

Documentation and Verification

• Government issued photo ID required
• Photo for badge (passport-style with solid background)
• Employment application required
• Work history verification (at least 2 past employments verified)
• References (2 references within 3 years required for Aya candidates)

State-Specific Requirements (New York)

• NYS Nurse Aide Registry (required within 3 months of start for certain care areas)

This position is ideal for licensed LPNs with recent hospital experience who are comfortable working night shifts and managing patient assignments independently with RN support. All required certifications, background checks, and health documentation must be completed timely to ensure compliance.
