Job Description

  • Position involves providing physical therapy services in a home health setting within Ventura County.
  • Home Health experience preferred.
  • California license preferred; candidates with pending CA license who have applied may be considered.
  • OASIS experience required; candidates without OASIS experience may be considered if they have strong home health experience.
  • Guaranteed hours based on completed visits: 25 visits equate to 40 hours.
  • COVID exemptions permitted.
  • Mileage reimbursement follows IRS standard rates.

Required Credentials & Documentation

Licenses & Certifications

  • California Physical Therapy License (preferred or pending with application submitted)
  • ACLS (AHA Instructor or Provider) – as applicable
  • BLS (AHA Instructor or Provider) – required for all
  • PALS, NRP, EFM – as applicable (check assignment details)
  • National Tech Certification Verification – as applicable
  • State License Verification (primary source online verification required)
  • NURSYS Verification (for RN, LPN/LVN, AP as applicable)

Background Checks (Annual Requirements)

  • Standard background check (7-year search including SS trace, county searches, nationwide, sex offender)
  • EPLS/GSA/SAM check
  • HHS/OIG check

Health & Immunization Requirements

  • Physical exam within 12 months of start
  • TDAP vaccine within past 10 years (no declinations accepted)
  • Negative TB test or chest X-ray within 1 year (with specific protocols for past positives)
  • Measles, Mumps, Rubella (MMR) proof or titer for candidates born in 1957 or later (no declinations accepted)
  • Hepatitis B titer or vaccine series (declination accepted if within 1 year)
  • Varicella immunity proof (titer, vaccines, or history of chickenpox; no declinations accepted)
  • Negative 9-panel drug screen within 1 year
  • Seasonal flu vaccination/documentation (declinations accepted)
  • COVID-19 vaccination or approved medical/religious exemption required prior to start, including booster dose

  • Government-issued photo ID (Driver’s License or State ID)
  • Valid passport copy before start
  • Photo for badge (passport style, shoulders up, front facing, plain background)
  • Personal or auto insurance with current expiration dates
  • Auto registration with current expiration dates
  • Signed acknowledgments for training content and hand hygiene policy
  • Two professional references
  • Annual skills checklist
  • Medication test/pharmacology competency exam (as applicable, minimum 80% score)
  • Competency exam (as applicable, minimum 80% score or Angoff score)
