Outpatient Clinic LVN/LPN Travel Contract

Overview

Location: West Hollywood, California
Employment type: Travel/Contract
Contract length: 13 weeks
Shift: Day, 3x8 hours, 8:00 AM–5:00 PM
Weekly hours: 24
Schedule notes: Starts Monday–Wednesday; may increase to 4x8 or 5x8 as caseload builds
Weekend work: Not required
Floating: Required, including rare floating to other specialty clinics
On-call: Not required
Scheduling requests: Not accommodated

Required Qualifications

Valid California LPN/LVN license
2 years of LPN/LVN experience
LPN/LVN experience within the last year
BLS certification issued by the American Heart Association or American Red Cross
IV certification
Blood withdrawal certification
Experience in an outpatient setting
Clinic experience
Experience with Epic
Must pass EPIC, sterile field, EVLT, vein ablation, and venipuncture screening criteria

Additional Requirements

Government-issued photo ID
2 professional references, including 1 supervisory reference
Physical exam within 1 year of start
Negative TB test or chest X-ray within 1 year of start
Drug screening within 1 year of start
Flu vaccination documentation required
MMR immunity or vaccination documentation required for measles, mumps, and rubella
Hepatitis B immunity, vaccination series, or declination required
Varicella immunity or vaccination documentation required
Fit test within 1 year, updated annually
Background check required
High school education verification required
Highest applicable education verification required
Pass/fail testing and competency exam required
Fire card required within the first 6 months of working onsite
Photo for badge required
Job description acknowledgement and policy attestations required
Module completion required before start
