Overview

• Travel/Contract role in Sylvania, Ohio
• Day shift: 5x8 hours, 8:00–16:30
• 13-week contract
• 4 weekends per contract as needed
• Adults and Geriatrics patient population
• Unit has 104 rooms and 104 beds
• Average daily census: 100

Required Qualifications

• Valid RN license in Ohio
• Associate of Science in Nursing / Associate Degree in Nursing
• Case Management (2 years of experience)
• Case Management experience within the last year
• Acute Hospital Case Management Experience
• Experience in a(n) Inpatient Rehab or Long Term Acute Care or Skilled Nursing setting
• Utilization Management
• Utilization Review (InterQual)
• Discharge Planning
• Experience with Epic
• NURSYS verification required for RN
• State license verification required
• ACLS
• BLS
• Dysrhythmia/EKG competency exam
• Medication Test/Pharmacology Competency Exam
• Competency Exam
• 2 professional references within 2 years of start, with at least 1 from a leader
• Skills Checklist 1
• Background check requirements include a 7-year background check, EPLS/GSA/SAM, HHS/OIG, education verification, and Ohio BCI/FBI fingerprint results within 12 months of start
• Health documentation required includes physical exam, fit test, negative TB or chest X-ray, Tdap, MMR immunity or exemption, Hepatitis B, Varicella, drug screen, and flu vaccination or exemption

Unit / Clinical Details

• Acute Care units include ICU, Acute Care neuro/Stroke, Acute Care Cardiology, Acute Care Oncology/Stroke, and Acute Care Post op/Orthopedic
• ICU procedures include ventilator management, invasive lines, and renal replacement therapy
• Acute Care procedures include cath lab, MRI/PET scan, endoscopy, and in-house dialysis
• Provider support includes interpretation services, IV teams, pharmacy, phlebotomy, physical therapy, radiology, rapid response team, respiratory services, social services, and transportation
• Provider coverage includes intensivist, NP, and hospitalist
• Staffing ratios: Days 1:16; Weekends 1:100
• Medication system: Pyxis
• Communication system: SBAR
• Business casual dress; no scrubs
• Unit accepts first-time travelers
