Position Details

• Specialty: Registered Nurse (Operating Room)
• Shift: Day shift, 5 days per week, 8 hours per day (7:00 AM – 5:30 PM)

Required Qualifications

• Valid Registered Nurse (RN) license in Kentucky
• Minimum of 1 year of Operating Room (OR) experience within the last year
• Advanced Cardiac Life Support (ACLS) certification
• Basic Life Support (BLS) certification issued by the American Heart Association
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Experience

• Minimum years of specialty experience: Not explicitly stated; assume standard RN experience for OR role

Identification & Legal

• Tetanus/Diphtheria/Tdap vaccination valid within 10 years (Td not accepted); declinations accepted if within 1 year of start
• Negative TB test via QuantiFERON blood test within 1 year prior to start (T-Spot/PPD not accepted)
• If positive TB, proof of chest X-ray within last 5 years plus TB questionnaire required
• Proof of immunity or vaccination for:

  • Measles (Rubeola) – 2 MMR vaccinations or positive titer
  • Mumps – 2 MMR vaccinations or positive titer
  • Rubella – 2 MMR vaccinations or positive titer
  • Varicella – 2 vaccinations or positive titer
  • Hepatitis B – series, titer, or declination within 1 year
• Seasonal flu vaccination or approved medical/religious exemption required during flu season (9/1–3/31) with timely submission of exemption requests

Drug Testing

• Drug test through 2020 lab within 30 days of start, including 9-panel with expanded opiates and oxycodone
• Annual drug test update required

Facility-Specific Documents

• Signed and dated specialty job description, renewed annually
• Employee health attestation and instructions form completed prior to start, including:

  • TB documentation upload
  • TB risk assessment completion
  • OSHA respirator medical evaluation completion
  • Flu vaccine or exemption documentation during flu season
• Fit test policy reviewed and acknowledged
• Badge instructions signed prior to start

References & Work History

• Two professional references within 1 year of start date required; one must be a supervisor or higher (Charge RN acceptable for one reference)
• Complete application/resume with education and work history
