Job Summary

Provide nursing care to frail and elderly participants both inside and outside the clinic setting under the direction of the provider and clinical nurse manager. Responsibilities include wound care, medication administration, point-of-care testing, and care coordination. Act as a liaison between participants, their families, providers, and the care team.

Contract Length

• 13 weeks

Work Schedule

• Monday to Friday, 8:00 AM to 4:30 PM
• May include varied schedules with evenings, nights, weekends, and overtime as needed
• Reliable transportation required

Key Responsibilities

Clinical Duties

• Perform and document wound care and other ordered procedures within scope of practice
• Administer medications and reinforce participant education regarding medications
• Process specialty pharmacy medication orders and escalate issues to provider as needed
• Coordinate medication changes and perform medication reconciliation during new participant visits
• Conduct monthly medication inventory (if applicable)

Care Coordination

• Manage pre- and post-operative clinical touchpoints, including participant follow-up and escalation as appropriate
• Monitor high-risk participants for vitals and medication adherence, escalating concerns as needed
• Conduct proactive quality check-ins for risk-stratified participants
• Communicate changes in participant condition promptly to the care team

General

• Complete all required documentation accurately and timely
• Maintain confidentiality and protect participant and team information
• Participate in quality improvement activities, training, and meetings
• Communicate regularly with supervisors and team members regarding workload and priorities
• Adapt to evolving business needs and perform other duties as assigned

Required Qualifications

• Valid LPN / LVN license in California
• LPN / LVN (1 year of experience)
• LPN / LVN experience within the last year
• Experience working with Geriatric patients
• Experience in a Clinic setting
• Care coordination experience
• Experience working with diverse backgrounds and cultures
• Skilled Nursing experience
