Overview

• Travel/Contract role in Home Health
• Location: San Ramon, California
• Start: ASAP
• Contract length: 13 weeks
• Shift: Day, 5x8 hours
• Guaranteed hours: 36 per week
• Case load: Home Health
• Area coverage includes Vacaville, Fairfield, Rio Vista, Suisun City, Dixon, Vallejo, Benicia, Napa, American Canyon, and Yountville
• May visit up to 6 patients per day
• Productivity standard: 27 points
• First-timers accepted
• Locals accepted
• On-call not required
• Weekends may be used to meet productivity
• Traveler must have their own transportation
• Occasional maximum radius of 50+ miles
• Timely communication and documentation required
• Documentation tools will be provided and must be returned

Required Qualifications

• Valid Occupational Therapist license in California
• Occupational Therapist (1 year of experience)
• Occupational Therapist experience within the last year
• BLS issued by American Heart Association or American Red Cross
• Experience in a Home Health setting

Preferred Qualifications

• 1 year of Home Health experience

Additional Requirements

• Personal or auto insurance must be current throughout the assignment
• Drivers license or state ID required
• Emergency contact form required prior to start
• Physical exam within 1 year of start, with clearance to work without restrictions or limitations
• Tdap within 10 years of start or annual declination
• Negative TB test or chest X-ray within 1 year of start
• Rubeola, Mumps, Rubella, and Varicella immunity documentation required
• Hepatitis B documentation required
• 10-panel drug screen within 1 year of start
• Flu vaccine or declination required year-round
• 2 professional references within 3 years of start
• Skills checklist required annually
• 1 year of employment verification required for all candidates
