Physical Therapist - Home Health (Travel/Contract)

Location: Salinas, California (Territory covers 50-mile radius including Santa Cruz/Watsonville)
Schedule: Monday to Friday, 8:30 AM – 5:00 PM (8-hour day with 30-minute lunch)
Contract Length: 13 weeks

Job Overview

• Provide physical therapy services in a home health setting, primarily geriatric cases
• Average caseload: 5-7 patients per day
• No on-call or weekend requirements
• Mileage reimbursed at IRS standard rate between patient homes
• Locals accepted at travel rate
• Experience with Homecare Homebase (HCHB) electronic medical records required
• OASIS experience required

Required Qualifications

• Minimum 1 year of home health physical therapy experience (first-time travelers not accepted)
• Valid California Physical Therapy license in hand (pending licenses not accepted)
• Basic Life Support (BLS) certification required
• Must meet all standard background checks and health documentation requirements (see below)

Certifications & Documentation

• BLS (Basic Life Support) certification required
• ACLS and PALS certifications preferred but not mandatory
• Government-issued photo ID (Driver’s License or State ID)
• Emergency contact form prior to start
• Annual background checks including 7-year search and federal exclusions
• Physical exam within 1 year of start date
• Immunization documentation: Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella (per client policy)
• 9-panel drug screen within one year of start
• Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31)
• Medication/pharmacology competency exam as applicable
• Annual skills checklist

Additional Information

• No prior travel experience required
• No weekend or on-call shifts
• Must be able to work within a 50-mile radius from the office hub
• Travel rate applies to local hires
