Job Description

• Provide primary and team nursing care, including care for primary stroke patients.

Required Qualifications

Licensure and Education

• Valid RN license in New York
• Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)

Experience

• Minimum 1 year of Medical/Surgical nursing experience
• Medical/Surgical nursing experience within the last year

Certifications (must be current and from American Heart Association)

• BLS (Basic Life Support)
• ACLS (Advanced Cardiovascular Life Support)
• 8 hours of stroke training

Health and Safety Requirements

• COVID-19 vaccine proof or agency declination (optional).
• MMR: Positive IgG titer or 2 vaccine series required; if negative/equivocal, vaccination must be completed within 30 days of start.
• Varicella: Positive IgG titer or 2 vaccine series required; if negative/equivocal, vaccination must be completed within 30 days of start.
• Hepatitis B: Positive antibody titer, 3 vaccine series, or specific declination form required.
• Influenza vaccine or specific declination form required; if declined, mask must be worn at all times on assignment.
• Tdap preferred but not required; if no record, specific declination form required.
• TB testing: Must have one of the following within required timeframes:

  • Two PPD skin tests (one within 12 months, one within 3 months) or
  • One Quantiferon/Spot test within 12 months.
  • For past positive TB tests, chest x-ray within 12 months may be required.
• Fit test for approved respirator models must be completed within the last 12 months prior to start.

Onboarding and Documentation Requirements

• Completed physical/health statement within the last 12 months including vitals, height, weight, Snellen eye chart, and physician clearance with no restrictions.
• Valid, unexpired U.S. driver’s license or state ID.
• Background check within 30 days of start including:

  • SSN address trace for all counties lived/worked in last 7 years
  • National and county criminal searches
  • FACIS 3 check
• Drug screen within 30 days of start: 9+ panel including amphetamines, barbiturates, benzodiazepines, cocaine, opiates, oxycodone, phencyclidine, methadone, methaqualone, propoxyphene, nitrites.
• Skills checklist for the assigned unit, fully completed and updated annually.
• Core competency exams for nursing (must score 80% or higher).
• Two professional references:

  • One supervisor reference from employment within last 12 months
  • One supervisor or peer reference from employment within last 3 years
• Employment verification of most recent employer.
• Resume accounting for all employment and gaps for last 7 years, including education history with degree titles and graduation dates.
• Signed and dated job description.
• Color headshot photo (professional, no filters, solid background).
• Completion of agency-specific forms including health personal information, code of conduct, confirmation of assignment, self-study packet and quiz, sexual harassment acknowledgement, parking registration form, and drug/tobacco acknowledgement.
• TB questionnaire and Epic questionnaire forms specific to the agency.

Additional Notes

• Extended travelers beyond one year must complete a health reassessment form.
• Candidates with Canadian licenses must provide proof of valid TN visa status or eligibility documentation.

This position requires a dedicated Medical/Surgical RN with stroke care experience, strong compliance with health and safety protocols, and thorough documentation for onboarding. The night shift and 15-week contract length offer a defined long-term opportunity.
