Location

Contract Length
- 13 weeks
Facility Setting
- Acute Rehab Unit
- Facility has 34 beds; the Case Manager will manage approximately 14 patients
- No floating to other units required
Work Schedule
- Monday to Friday
- 8:00 AM to 4:30 PM
- Approximately 32 hours per week
Required Experience
- Minimum 1 year of case management experience in a hospital setting
Electronic Medical Record (EMR) System
- Cerner
Dress Code

- Employee parking available in East lot
Required Qualifications
- Valid RN license in Michigan
- Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
- Case management experience
