- 15 RNs
- 15 Social Workers
- Patient Ratios: 1 RN to 30 patients
- Equipment: Computer, office equipment
- EMR: Meditech / Midas
Requirements
Must-Have Skills
- Meditech or HCA experience
- At least 1 year of bedside case management experience
- Knowledge of post-acute care
Preferred Skills
- Experience with Meditech/Midas or HCA
- Throughput experience
Responsibilities
- Discharge planning and progression
- Conducting difficult conversations with families and patients
- Collaborating with MDs on plans of care

- Adaptable and proactive
Dress Code
- Scrub Color: Navy with white lab coat
- Business casual with white lab coat
- Closed-toe shoes (no crocs, no fake nails)
