Job Opportunity: Virtual Care Coordinator

Location

  • New York, NY

Employment Type

  • Contract
  • Part-time: 3 days per week
  • Evening shift every weekend (nonnegotiable)
  • Approximately 7 hours per day, 21 hours per week
  • Contract length: 13 weeks

Job Summary

The Virtual Care Coordinator supports virtual patient care activities and promotes patient-focused care through remote monitoring and telehealth services. Responsibilities include patient safety monitoring, telehealth platform support, collaboration with clinical teams and technology partners, quality assurance, and training.

Key Responsibilities

  • Perform quality assurance and peer reviews of telehealth and virtual patient observation programs.
  • Collaborate with leadership to evaluate program metrics and recommend improvements.
  • Lead case reviews and implement solutions with hospital partners.
  • Triage and assign cases for virtual monitoring.
  • Develop and implement telehealth policies and workflows.
  • Maintain operational databases tracking telehealth program performance and quality metrics.
  • Identify trends and participate in corrective action plans.
  • Provide training on telehealth workflows, platform use, and documentation.
  • Serve as liaison between clinical staff, physicians, technology partners, and vendors.
  • Facilitate virtual patient care by analyzing utilization reports and coordinating patient transitions.
  • Provide real-time support to troubleshoot telehealth and remote monitoring equipment issues.
  • Continuously observe assigned patients for clinical status changes, safety, and fall prevention.
  • Monitor early warning signs such as vital signs and catheter days.
  • Communicate effectively with patients, visitors, and clinical teams.
  • Report any clinical or behavioral concerns immediately to nursing staff or physicians.
  • Document patient behavior, interventions, and escalate high-risk situations.
  • Monitor telehealth consult orders and ensure sessions start and complete as scheduled.
  • Maintain patient privacy in compliance with HIPAA and organizational policies.
  • Escalate patient safety concerns and equipment issues appropriately.
  • Maintain accurate records of interventions and safety events.
  • Participate in staff meetings, team briefs, and maintain punctuality and attendance.
  • Maintain a safe, clean, and functional work environment.

Required Qualifications

  • Minimum 2 years of Care Coordinator experience, including experience within the last year
  • Experience as a Monitor Technician
