Licensed Practical Nurse (LPN)
Location: Brooklyn, New York
Employment Type: Staffing - Supplier Sourced
Contract Length: 13 weeks (88 days ≈ 13 weeks)
Job Overview
- Provide nursing care under the direct supervision of a Registered Nurse or physician.
- Perform nursing duties requiring tact and judgment in patient care and treatment.
- May assist in supervising non-professional staff.
- Follow nursing department philosophy, standards, objectives, and policies.
Responsibilities
- Demonstrate proficient knowledge in treating patients requiring nursing care.
- Use diagnostic results and other information to evaluate patient condition accurately.
- Apply knowledge of illnesses, injuries, and diseases in nursing processes.
- Perform timely, comprehensive, and accurate patient reevaluations and documentation, especially when significant changes occur.
- Respect patient privacy during evaluations.
- Contribute to individualized care plans with measurable goals and update plans as patient conditions change.
- Monitor patient outcomes and ongoing care needs, including discharge planning and coordination of post-institutional resources.
- Collaborate with Care Managers, physicians, and other clinical staff to plan timely interventions for patients and families.
Required Qualifications
- Licensed Practical Nurse (LPN) license valid in New York.
- Experience in Long-Term Care (LTC) or nursing home settings is required.
- Strong professionalism and communication skills.
- EPIC electronic health record experience is a plus.
Physical and Work Environment Requirements
- Ability to work in environments with dust, noise, and potential exposure to mechanical, electrical, radiant, explosive, burn, and chemical hazards.
- Ability to hear, talk, walk, stand, bend, stretch, and sit for extended periods.
- Ability to read, write, and interpret documents.
- May be required to handle, dispose of, and identify hazardous waste/materials.
- May assist in lifting patients onto/out of exam beds or procedure stations.
- Routine exposure or potential exposure to blood, body fluids, and tissue.
- Must be trained and knowledgeable about hazardous waste/materials policies and maintain applicable qualifications.
- Knowledge of emergency codes and procedures is required.
Shifts Available
- All shifts needed, including:
- Nights with every other weekend (11:00 PM – 7:15 AM)
- Evenings with every other weekend (3:00 PM – 11:15 PM)
- Days with every other weekend (7:00 AM – 3:15 PM)
- Weekend-only roles available
