BLS issued by American Heart Association
Associate of Science in Nursing / Associate Degree in Nursing
Government issued photo ID
Proof of E-Verify or attestation
7-year criminal background check covering all locations lived/worked in the past 7 years, including international searches
EPLS/GSA/SAM
HHS/OIG
Highest degree completed, with either transcript/diploma or education verification accepted
Physical exam within 1 year of start, updated annually
Fit test within 12 months of start, updated annually
Tdap within the last 10 years
Negative TB screening or chest x-ray
Positive titer or 2 vaccines for measles, mumps, rubella, and varicella
Positive IGG Hep B quantitative titer within 5 years of start, or facility declination
11-panel drug screen including cotinine or nicotine and marijuana within 30 days of start
Flu vaccination or approved exemption during flu season
Eye exam within 6 months of start
Color blind exam within 5 years of start
