Job Details
- Shift: 4x10 hour shifts
- Call: Rotating call required
- Staffing Ratio: 1:1
- Unit Focus: GVHD, BO, CTCL, Photopheresis
- Special Equipment: CellEx
- Skillset Requirements:
- Minimum 3 years of experience in dialysis, apheresis equipment, central lines, and ports
Required Certifications
- BCLS (Basic Cardiac Life Support)
- ACLS (Advanced Cardiac Life Support) - AHA or RQI accepted
- BLS (Basic Life Support) - AHA or RQI accepted
- PALS (Pediatric Advanced Life Support) - AHA or RQI accepted
- NRP (Neonatal Resuscitation Program) - AHA only
- Chemotherapy certification (as applicable)
- TNCC (Trauma Nursing Core Course) (as applicable)
- Electronic Fetal Monitoring (as applicable)
- NIH Stroke Certification (as applicable)
- Medication/Pharmacology Competency Exam
- Color Blind Testing (Ishihara exam or equivalent, 100% minimum score)
- Unit Competency Exam
- Orientation Safety Manual Acknowledgement (HIPAA)
- State Nursing License verification (Indiana RN license required, must be current and verified within 30 days before start)
Required Documents & Health Screenings
- Valid Indiana RN license and certifications in hand before submission
- Physical exam completed within 12 months of start date
- Fit mask test within 12 months (approved respirators only)
- Tuberculosis screening (PPD, QuantiFERON-TB Gold, or T-SPOT within 1 year)
- Immunizations or titers for:
- Rubeola (Measles)
- Mumps
- Rubella
- Varicella (no exemptions accepted)
- Hepatitis B (series, titer, or signed declination)
- Tetanus/Diphtheria/Tdap (preferred but not required)
- Negative 10-panel drug screen within 12 months
- Seasonal flu vaccination or approved medical/religious exemption (declinations not accepted)
- Color Blind Exam (Ishihara or equivalent)
Background & History Requirements
- Background checks including SSN trace, county, statewide, national sex offender, FACIS Level III, education verification, and international search if applicable
- Minimum 2 professional references from recent employment verified within 2 years (must be from charge, manager, supervisor, or above)
- Current employment application with full work history and explanations for any gaps over 30 days
- Skills checklist completed within 12 months and annually thereafter
Requirements/Qualifications
- Valid Registered Nurse (RN) license in Indiana
- Minimum of 3 years of dialysis experience, including experience within the last year
- Experience with dialysis equipment, apheresis, central lines, and ports
- Basic Life Support (BLS) certification issued by the American Heart Association
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
