Occupational Therapist - Outpatient (Travel/Contract)

Location: Pleasant Prairie, Wisconsin
Contract Length: 13 weeks

Job Details

• Shift:

  • 35 hours per week
  • Monday, Wednesday, Friday: 7:00 AM – 5:30 PM (10 hours)
  • Tuesday: 12:30 PM – 5:30 PM (5 hours)
• Start Date: July 13
• Locals Accepted: Yes (at a lower rate)
• First Time Travelers: Not accepted

Required Qualifications

• Valid Occupational Therapist license in Wisconsin
• Minimum 1 year of Occupational Therapist experience within the last year
• Completion of at least one travel assignment
• Basic Life Support (BLS) certification

Background and Compliance Requirements

• 7-year background check including Social Security trace, county searches for all locations lived/worked/schooled in the last 7 years, nationwide search, and sex offender registry
• Professional references: 2 verified within 2 years of start
• Annual skills checklist completion

Certifications and Competencies

• BLS (Basic Life Support) – ARC or AHA accepted; Provider, Instructor, and RQI acceptable
• Other certifications (ACLS, PALS, NRP, NIH Stroke Certification) may be required depending on specialty but not specified as mandatory for this role
• Medication test/pharmacology competency exam as applicable
• Color blind testing if applicable to specialty (not specifically required here)

Health and Immunization Requirements

• Physical exam within 1 year of start
• Negative QGold TB test within 1 year (TSpot/PPD not accepted) or negative chest X-ray within 60 days of start
• Immunizations or positive titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Varicella
  • Hepatitis B (titer, series, or declination)
• Seasonal flu vaccination or approved medical/religious exemption required during flu season (October to March)
• Negative 10-panel drug screen with expanded opiates within 14 days of start (marijuana positive not disqualifying)

Facility Specific Requirements

• Photo for badge (passport-style, shoulders up, no filters)
• Confidentiality statement signed prior to start
• Compliance checklist completed by agency representative

• This is a travel/contract position with a defined 13-week duration
