Registered Nurse – Outpatient Surgery/Day Surgery
Location: Ithaca, New York
Employment Type: Long-Term Assignment (13 weeks)
Job Description
- Role involves Preoperative (Preop) and Post-Anesthesia Care Unit (PACU) nursing.
- On-call requirement: approximately 3-4 weeknight calls and one 24-hour weekend call within an 8-week period.
- Weeknight call hours: 9 PM – 7 AM
- Weekend call hours: 7 AM – 7 AM
- Shift options include:
- 6 AM – 6 PM
- 7 AM – 7 PM
- 8 AM – 8 PM
- 9 AM – 9 PM
Required Qualifications
- Valid RN license in New York.
- Minimum 2 years of Day Surgery experience, including experience within the last year.
- Experience in Outpatient Surgery.
- BLS certification issued by the American Heart Association, valid throughout the assignment.
- PALS certification issued by the American Heart Association, valid throughout the assignment.
- ACLS certification, valid throughout the assignment.
- Associate of Science in Nursing or Associate Degree in Nursing.
Health and Safety Requirements
- Immunizations and Titers:
- MMR: Positive IgG titer or 2 vaccine series required; if negative/equivocal, vaccination series must be completed within 30 days of start.
- Varicella: Positive IgG titer or 2 vaccine series required; if negative/equivocal, vaccination series must be completed within 30 days of start.
- Hepatitis B: Positive antibody titer, 3 vaccine series, or specific declination form required.
- Tdap: Preferred within last 10 years; if not available, specific declination form required.
- TB Testing:
- Must have one of the following:
- Two skin PPD tests (one within 12 months, one within 3 months)
- One Quantiferon or Spot test within 12 months
- Past positive TB tests require chest X-ray within 12 months.
- Fit Test: Must be completed within the last 12 months prior to start; annual requirement. On-site fit testing available if needed.
- Physical/Health Statement: Must be from within the last 12 months, including height, weight, vitals, Snellen eye chart, and a statement of clearance with no restrictions from an approved provider (MD, NP, PA, DC, or DO).
- COVID-19 Vaccine: Optional; upload proof of vaccine or agency declination.
Professional Requirements
- Valid US driver’s license or state ID (cannot be expired).
- Background check within 30 days of start, including:
- SSN address trace report covering all counties lived/worked in last 7 years
- National and county criminal searches
- FACIS 3 check
- Drug screen within 30 days of start: 9+ panel including amphetamines, barbiturates, benzodiazepines, cocaine, opiates, oxycodone, phencyclidine, methadone, methaqualone, propoxyphene, nitrites.
- Two professional references required:
- One supervisor reference from employment within last 12 months
- One supervisor or peer reference from employment within last 3 years
- Employment verification of most recent employer required (agency employment counts as one employer).
- Signed and dated job description acknowledgment.
- Completion of required professional documentation and acknowledgments, including:
- Personal information form
- Code of conduct
- Confirmation of assignment form
- Self-study packet and quiz
- Sexual harassment acknowledgment
- Parking registration form
- Color headshot photo (straightforward, solid background, no filters or sunglasses).
Documentation Required at Submission
- Resume covering all employment and gaps for last 7 years (or back to graduation), including education history with degree title, graduation date, and locations.
- Completed Epic questionnaire (specific form required).
