Physical Therapist - Home Health

Location: Petaluma, California
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Details

  • Schedule: Monday to Friday, 8:30 AM to 5:00 PM (30-minute lunch)
  • Average Patients per Day: 5-7
  • Patient Population: Geriatric home health cases
  • Territory: Covers a 50-mile radius from the office/hub
  • Mileage reimbursed at IRS standard rate between patient homes
  • Electronic Medical Record (EMR) system: Experience required with HCHB
  • OASIS experience required

Required Qualifications

  • Valid Physical Therapist license in California
  • Minimum 1 year of Physical Therapist experience within the last year
  • One completed travel assignment
  • Experience in a Home Health setting
  • Experience with OASIS
  • Basic Life Support (BLS) certification

Additional Information

  • No on-call or weekend requirements
  • Locals accepted at travel rate
  • Shift: Day shift, 40 hours per week (5 days x 8 hours)
