Physical Therapist - Home Health

Location: Glen Carbon, Illinois
Employment Type: Travel/Contract
Contract Length: 13 weeks
Shift: Monday to Friday, 8:00 AM – 4:00 PM (5x8)
Hours per Week: 40 hours guaranteed
Daily Caseload: 5-6 units


Job Description

  • Provide physical therapy services in a home health setting.
  • Patient population includes geriatrics and middle-aged adults; no pediatric patients.
  • Common patient diagnoses: orthopedic, cardiac, neurological, pulmonary, diabetes, and others.
  • Work within a multidisciplinary team.
  • Responsibilities include case management, supervising PT assistants, and delivering patient-centered care.
  • Emphasis on professionalism, communication skills, flexibility, positive attitude, and quality care.
  • No on-call or floating requirements.
  • Locals and candidates with pending Illinois state licenses are accepted.
  • Orientation provided with a preceptor.
  • Charting system: Epic.
  • Scrub color and parking provided.

Required Qualifications

  • Valid Physical Therapist license in Illinois.
  • Minimum 1 year of Physical Therapist experience, including experience within the last year.
  • Experience in a Home Health setting.
  • Basic Life Support (BLS) certification (AHA only).
  • Case management skills.

Compliance and Background Checks

  • Criminal background check within 6 months required (7-year search including statewide and nationwide checks).
  • Quarterly EPLS/GSA/SAM and HHS/OIG checks required.
  • Education verification of highest related degree required before start.
  • Fingerprinting not required for Illinois home care but required for other Illinois long-term care or rehab assignments.
  • State-specific exclusion and sanction list checks required for Illinois.
  • Missouri-specific background checks apply if applicable.

Certifications & Competencies

  • BLS certification (AHA only).
  • Other certifications (e.g., ACLS, PALS, NRP) not required unless specified by unit.
  • Unit-specific competency exams required and must be updated annually.
  • Medication/pharmacology competency exam required as applicable.
  • Crisis Prevention Institute (CPI) or Management of Assaultive Behavior (MAB) certifications if applicable.

Health Requirements

  • Physical exam within 1 year of start.
  • Negative TB test or chest x-ray within required timeframes.
  • Required immunizations or proof of immunity for:
    • Measles (Rubeola)
    • Mumps
    • Rubella
    • Varicella
  • Tdap vaccine or declination required (expires every 10 years).
  • Seasonal flu vaccination or approved exemption required during flu season.
  • Drug screening within 1 year of start, including a comprehensive panel.
  • Fit mask testing required annually; must be able to pass fit test with acceptable N95 models.

Facility Policies & Documents

  • Must sign HIPAA acknowledgement, confidentiality form, latex allergy questionnaire, hand hygiene, fingernail, and dress code policies prior to start.
  • Regulatory compliance education quiz required annually.
  • Badge photo required (head and shoulders, white background, no obstructions, taken within 6 months).
  • Employment application and professional references required.
  • Skills checklist specific to unit required within 1 year of start.
  • Orientation modules completed through online platform; approximately 16 hours.

Additional Information

  • Every other weekend work required per facility policy.
  • Requests for time off limited to no more than 7 days; approval required.
  • No floating to sister facilities.
  • Large parking space available at office.

References

  • Two professional references required; supervisors within the last year preferred.
  • For recent graduates, references from clinical directors or preceptors accepted.

This position is suitable for first-time travel therapists possibly after interview.
