Job Details

• Productivity requirement: 30 points
• Mileage reimbursement is provided; mileage begins after the first patient to the last patient
• Counties covered: Clay, Cleburne, Randolph, Tallapoosa

Responsibilities

• Perform physical therapy evaluation and treatment based on physician orders/referrals
• Complete comprehensive evaluations using appropriate assessment tools and procedures
• Review patient records for diagnosis, medical history, precautions, and contraindications
• Establish measurable goals and develop treatment plans
• Reassess treatment effectiveness, patient progress, and need for plan changes
• Communicate patient needs and progress to the treatment team, physician, and family members as applicable
• Consult with medical providers regarding evaluation, treatment, progress, and discharge
• Recommend and assist in securing adaptive equipment to improve function
• Maintain evaluation, treatment, and billing documentation as required
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities
• Participate in staff development programs and meetings and maintain continuing education as required
