Schedule

• Monday through Friday, 8:30 AM to 5:00 PM (40 hours per week)
• Must be willing and able to float to sister facility and other departments as requested and qualified

Job Description

• Provide direct clinical social work services, including counseling, discharge planning, and case management for hospital patients and their families
• Assess and document emotional, social, financial, environmental, and follow-up referral needs
• Collaborate with interdisciplinary healthcare team to formulate and monitor psychosocial plans of care tailored to patient strengths and needs
• Develop discharge plans and identify barriers to meeting estimated length of stay
• Facilitate integration with community-based resources to optimize patient health
• Support quality improvement, clinical research, and community networking efforts
• Provide advocacy and integrated mind-body-spirit care to patients and families
• Complete required documentation in medical records and electronic systems
• Maintain constructive relationships with team members, external providers, and management

Required Qualifications

• Valid Licensed Clinical Social Worker (LCSW) license in California
• Minimum of 1 year of Licensed Clinical Social Worker experience within the last year
• Skilled in counseling patients and families coping with loss and grief
• Experience in advocacy for patients and families
• Ability to conduct advanced directive conversations with end-of-life patients and their families
