Therapy PT Home Health Travel/Contract Job - Georgia (Newnan)

Job Details

• Schedule: Monday to Friday, 8:00 AM - 5:00 PM (some positions may have 4x10 shifts Friday-Monday)
• Patient Visits: Home visits to private residences and assisted living facilities (ALFs)
• Average Patients per Day: 5-6
• Productivity Expectation: 30 points per week (points assigned based on visit type and activities)
• Travel Requirements: Must be willing to travel up to 75 miles from home clinic; daily travel approx. 35-45 miles, weekly 170-200 miles depending on contractor location
• Guaranteed Hours: No (may be floated to nearby clinic if patient census is low)
• On-Call: Yes, approximately 1 weekend per month (varies by location)
• Local Candidates: Accepted at the same rate

Experience & Certifications

• Required Experience: Minimum 1 full year of recent Adult Home Health experience with OASIS documentation experience
• First-Timers: Not accepted; must have recent Home Health experience
• Certifications Required: CPR and BLS (Basic Life Support)
• Licensure: Must have license in hand (pending licenses accepted only if license issuance takes a few days)

Documentation & Systems

• EMR Systems: HCHB for documentation, OASIS, Forcura
• Mileage Reimbursement: IRS standard rate; compensated from patient home to patient home (commute to/from first and last patient not billable)

Coverage Area

• Troup County, GA (including Hogansville, LaGrange, West Point)

Additional Requirements & Compliance

• Must have reliable transportation and current driver's license
• Agency-required liability insurance required
• Background checks, drug screening, and health documents (physical exam, immunizations, TB test, COVID vaccination or declination, etc.) required per Aya Healthcare standards
• Various state-specific background checks and fingerprinting may be required depending on placement state
• Candidates must complete compliance training, confidentiality agreements, HIPAA and privacy policies, and other facility-specific documents prior to start
• References: Two references required for all placements
• Work history verification: Must verify at least two most recent employers or last 7 years if only one employer
