Job Opportunity: Case Manager (Registered Nurse)

Location: Fort Lauderdale, Florida

Contract Length: 13 weeks

Job Description

• Manage case load with a ratio of approximately 1:25-30 adults and geriatrics.
• Work schedule: Monday to Friday, 8:30 AM to 5:00 PM (5 days a week, 8-hour shifts).
• One full weekend shift every 4 weeks.
• No call or holiday shifts required.
• Perform initial patient assessments and follow-up visits at least every third day.

Required Qualifications

• Valid RN license in Florida.
• Minimum 2 years of case management experience, including experience within the last year.
• Acute hospital case management experience in an acute care setting.
• Experience in discharge planning.
• Experience with Epic electronic health record system.
• NIH Stroke Scale (NIHSS) certification or competency.
• Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).

Certifications & Training Requirements

• Basic Life Support (BLS) certification (AHA or ARC).
• Advanced Cardiac Life Support (ACLS) certification if applicable to unit.
• NIH Stroke Certification required for all RNs.
• Completion of facility-specific online orientation and training modules, including but not limited to:

  • Privacy Policy, AHCA Affidavit, Demographic Information Sheet
  • ICU Medical modules related to medication and equipment
  • Nursing-specific modules such as IV medication administration, impairment recognition, hazardous drugs handling, stroke core measures, end-of-life care, procedural sedation, and others as assigned
  • Annual Risk Management, Compliance & Ethics, Code of Conduct, Emergency Management, Harassment Prevention
• Annual fit testing for approved masks (3M 1860 Regular or Small).
• Annual TB questionnaire and negative QuantiFERON lab test within 6 months of start (TSPOT and PPD not accepted).
• Immunization or proof of immunity for:

  • Measles (Rubeola), Mumps, Rubella (2 MMR vaccines or positive titer; boosters or declination accepted)
  • Hepatitis B (series, titer, or signed declination)
  • Varicella (2 vaccines, positive titer, or declination)
• Negative 10-panel drug screen within 12 months of start (instant tests not accepted).
• Flu vaccination or declination required during flu season (Oct 1 - Mar 31), with documentation due by December 1st.

Compliance & Documentation

• Government-issued photo ID must be carried during work hours.
• Background check within 6 months of start, covering all locations lived/worked/schooled in the past 7 years, including aliases, SSN trace, nationwide and sex offender checks.
• Education verification required; official transcripts or diploma accepted if verification is unavailable.
• Florida AHCA Level II fingerprinting clearance required prior to start; must be updated every 5 years.
• Physical exam within 6 months of start confirming fitness for duty without restrictions, signed by DO, MD, NP, APRN, or PA.
• Two professional references required, with at least two from the last 2 years.
• Annual skills checklist and competency exams as applicable.

Additional Information

• Local candidates within a 60-mile radius are considered.
• This position requires no call or holiday shifts.
• The role involves working with adult and geriatric populations in an acute care hospital setting.

Work Hours

• 40 hours per week, 8 hours per day, day shift (8:30 AM - 5:00 PM).
• One full weekend shift every 4 weeks.
