Description

American Traveler is seeking an experienced RN Case Manager for an acute care hospital position in Broward County, FL, requiring at least 2 years of recent hospital case management and discharge planning experience.

Details

• Acute care hospital setting, Case Management unit
• Adult and geriatric patient population
• Caseload ratio of 1:25–30 patients
• Schedule: 5x8-hour day shifts, 8:30 AM–5:00 PM, Monday–Friday, plus one full weekend every 4 weeks
• No on-call or holiday requirements
• EMR systems in use include Epic, AllScripts, PowerChart, and CarePort — proficiency in all is required
• Microsoft Office Suite proficiency required
• 13-week contract

Requirements

• Active FL or Compact RN license currently in hand
• Current BLS certification required at time of consideration
• Minimum 2 years of recent, consecutive acute care hospital case management experience within the past 24 months required
• Discharge planning experience required
• Level 1 Trauma experience preferred
• Degree must be from an accredited school, or from an institution approved by the Florida Board of Nursing (FBON)
• Proficiency in Epic, AllScripts, PowerChart, CarePort, and Microsoft Office Suite required
• Proof of vaccination status required — declinations not accepted

Additional Information

• Responsible for completing initial patient assessments and reassessing patients at a minimum of every third day
• Facility is a smoke-free environment
• Local candidates within a 60-mile radius of the facility are considered, though a lower pay rate applies for local candidates
• No Basic Arrhythmia assessment is required for this Case Management role
