Skills checklist updated annually (as applicable)
Radiation badge provided by facility for new radiology-related candidates (including Cath Lab Tech)
Attestation of prior radiation exposure required for new candidates in listed units including Cath Lab Tech
Clear, passport-style photo required for badge (recent, professional attire, neutral background)
Facility-specific forms and checklists must be completed prior to start
Maryland CJIS/DSS/FBI fingerprinting required for behavioral health units (not applicable here unless assigned)

Candidates must comply with fit mask testing and facial hair policies; no exceptions without approved accommodation forms

No floating or reassignment outside scope of practice
