Licensed Practical Nurse (LPN) – Medically Ready for Discharge Unit

Location: Klamath Falls, Oregon
Contract Length: 13 weeks
Shift: Nights
Hours: 12 hours per shift, approximately 36 hours per week


Job Summary

The LPN works under RN supervision to provide direct patient care for patients medically ready for discharge but experiencing delays due to various barriers. This role involves collaboration with a multidisciplinary team and includes assigning and supervising tasks to LPNs, CNAs, and unregulated assistive personnel. The LPN applies nursing principles and practices within the scope of practice for LPNs in Oregon.


Essential Job Functions

  • Perform focused assessments and implement prioritized plans of care as directed by the RN.
  • Evaluate patient responses and progress toward expected outcomes.
  • Assign and supervise interventions to LPNs, CNAs, or UAPs based on licensure and patient needs.
  • Collect patient data outside RN plan parameters and communicate findings to the RN.
  • Accurately document assessments, interventions, patient responses, and progress in the electronic health record.
  • Assist with patient and family education related to discharge plans, medications, and follow-up care.
  • Communicate effectively with the interdisciplinary team to ensure continuity of care and smooth transitions.
  • Collaborate with RNs, case managers, social workers, and other team members to support discharge planning.
  • Adhere to hospital policies, safety protocols, and participate in quality improvement initiatives.
  • Demonstrate cost-conscious use of supplies and equipment.

Required Qualifications

  • Valid LPN/LVN license in Oregon
  • Minimum of 2 years LPN/LVN experience
  • LPN/LVN experience within the last year
  • BLS (Basic Life Support) certification
  • Medical-Surgical (Med Surg) nursing experience

Physical and Mental Demands

  • Good general health.
  • Ability to frequently stoop, bend, reach, stand, walk, and sit.
  • Ability to lift at least 50 pounds and push/pull hospital equipment.
  • Ability to function effectively during periods of stress, trauma, or disease.

Additional Information

  • 12-hour night shifts, every other week schedule required.
